At Palmeira Home Health, this thorough interview and evaluation is part of our coordinated approach to managing your overall health status. Our evaluation focuses on educating you in self-care management and partners with you and your doctor to promote disease prevention and proactive care – which includes the family or caregivers.

This team approach facilitates an easier transition for you from a hospital, rehab or nursing facility to your home, and it actively engages you and your caregivers in your health care.

Yes. After your doctor refers you to Palmeira Home Health Care, a clinician will come to your home to assess your needs. We will communicate with your doctor to discuss the assessment and work together to develop your personal plan of care. Our staff will implement your physician-ordered plan of care and keep your doctor updated about your progress. If your condition or your needs change, we’ll collaborate with your doctor to review your plan of care and make any adjustments deemed necessary.

The frequency of home health care visits and the services provided are based on your doctor’s orders and your personal plan of care. Your doctor may change your plan of care, increasing or decreasing the number of visits or services provided, in order to provide you with the best home health care for your needs.

Absolutely! And we’d be honored to care for you or your loved one. Federal law gives patients with Medicare the freedom to choose their health care provider.

Medicare patients requiring home health must have an order by a physician, and include an accompanying statement of homebound status.

When we receive a referral from your physician we will contact your insurance regarding requests, authorizations, determining co-payments and the number of visits they will approve. Once we have received all information, we will contact you with a complete update prior to starting care.

If you meet certain eligibility requirements, Medicare may pay for your covered home health care for as long as you’re eligible and your doctor certifies that you need it. Additionally, your insurance may also cover home health care. We will ask for an authorization from your insurance company to learn about their specific eligibility requirements and follow up with you on their answers.

A patient’s residence is wherever he or she makes their home. This may be a house, an apartment, a relative’s home, an assisted living or group home.

Hospitals, skilled nursing facilities (SNF) or intermediate care facilities (ICF), like a rehab, are not considered the patient’s home.

PCA agencies perform household and personal care services, like preparing meals, cleaning, and helping with bathing or dressing – Medicare does not cover these types of services.

Skilled home health care provides medical treatment for an illness, injury, or post-operative care, all with the goal of helping you recover, regain your independence and become as self-sufficient as possible.

Palliative Care can be provided at any age and at any stage in a serious illness and can be provided along with curative treatment and, yes, the palliative care plan can be followed while under Palmeira Home Health care.

• Typically, a consult to palliative care is initiated while a person is hospitalized and it is a specialized form of medical care for people with a serious illness.
• Palliative care focuses on providing patients with relief from the symptoms and stress of a serious illness. The goal is to improve quality of life for both the patient and the family.
• Home based Palliative Care is not available everywhere and is being promoted to the United States Legislature by the Center to Advance Palliative Care (org) and the National Hospice and Palliative Care Organization (NHPCO.org) to be included as a home based program.
• Hospice care is designed to provide non-curative treatment and comfort for those who are facing a life-limiting illness. It offers a support system of medical, social, psychological, and spiritual services that support a patient, their family and other loved ones.
