CHF After Care
A healthy heart can pump blood to all parts of the body in a few seconds. When the heart can no longer do this, you have heart failure. Blood that should be pumped out of the heart backs up into the lungs and other parts of the body. This causes symptoms of heart failure—shortness of breath or swelling in the abdomen, hands, legs and feet.
Many people with heart failure have fluid buildup in their lungs (congestion). So, heart failure is often called CHF (congestive heart failure).
Heart failure can range from mild to severe. Most often the symptoms can be controlled with medicines, rest, and diet. When heart failure symptoms are found early and treatment is started, a person with heart failure can lead a more normal life.

COPD After Care
COPD refers to many different obstructive lung diseases.
The most common lung diseases are:
- Chronic Bronchitis
- Emphysema
- Asthma
- Bronchiectasis
- Cystic Fibrosis (CF)
When you have COPD, there may be changes in your lungs. They may:
- Make more mucus
- Have less oxygen and may have more carbon dioxide
- Have less working space to hold air
When these changes occur, they may cause these problems:
- Shortness of breath
- Wheezing
- Coughing
- Too much mucus in the lungs
- Tightness in your chest
Living with COPD is a challenge for many – especially after the onset of the disease. That’s why Palmeira Home Health as developed a comprehensive program specifically designed to help patients with COPD recover from an initial illness and learn to manage the condition. Your nurse (Registered Nurse or Licensed Vocational Nurse) and therapist (physical or occupational) will work with you to build a comprehensive care plan designed to measure your lung function, set goals, and work through meeting those goals for your better outcome.
Living with chronic lung disease means learning to control it, and the best way to control your disease is to be an active partner in your treatment.
Practice daily what you learn from your health care team and the material they give you. Ask your care team any questions you may have. Then, when breathing problems or infections occur, you will know just what to do. You will be in control and feel good about yourself.
